Memory Care Assisted Living Overview

Memory Care Assisted Living at the Lutheran Home provides residential and supportive services to older adults with Alzheimer’s and other forms of dementia. Our goal is to keep residents engaged in stimulating activity programming while maximizing their safety in a secure environment. Each resident in Memory Care will have a personalized plan of care based on an individualized assessment. Memory Care is staffed by a licensed nurse 24 hours per day, seven days per week, so you can rest assured that your loved one’s needs are met.

What’s Included

Lutheran Home Memory Care Assisted Living offers an “all-inclusive” rate which includes all usual and customary supplies. Your monthly service fee includes:

  • 24-hour staffing with specialized training in medication administration, fire safety, and dementia care
  • Health monitoring by clinical nursing staff
  • Medication management
  • Secure environment with wander management technology
  • Structured daily activities including physical and memory exercises, social events, art classes, music, community outings, games, pet therapy and intergenerational programming
  • Three meals per day planned under the supervision of a registered dietitian and snacks as desired
  • Utilities (heat, electricity, water, and cable TV)
  • Housekeeping services
  • Laundry services

What’s Not Included

  • Ancillary items and items billed to Medicare such as medications and diabetic supplies are billed separately.
  • Supportive services such as dental, vision, podiatry, geropsych and audiology are also billed separately.
  • Beauty shop and Courtyard Café purchases are billed separately.

PROGRAMMING

Structured daily activities including physical and memory exercises, social events, art classes, music, community outings, games, and pet therapy are just a few of the activities that occur on a regular basis.  

In addition to all our regularly scheduled activities and outings, we provide unparalleled intergenerational programming. Children from the Lutheran Home Children’s Center enjoy various activities with their grandfriends on a regular basis. Intergenerational programming promotes educational objectives, arts and recreation interests, desired states of health and welfare, and religious and spiritual well-being. Furthermore, intergenerational programs have been found to diminish ageist stereotypes, strengthen communities, and lead to improved services for children, youth and older adults.

STAFF & VOLUNTEERS

Staffing

  • Nursing staff
    • Our memory care assisted living nursing staff assesses residents on an ongoing basis. Nurses help create individualized care plans, monitor resident’s health and evaluates the continued appropriateness of each plan.
  • Resident Assistants
    • Resident Assistants (RAs) assist residents with activities of daily living, which might include using the toilet, bathing, washing, dressing, serving food. They also help with other tasks such as recreational and social activities.
  • Physicians
    • The Lutheran Home has physicians who can follow resident care in the Memory Care Assisted Living. Their role is to monitor the overall health of the residents they are assigned to.
  • Optum Nurse Practitioner
    • If your loved one takes part in UHC’s Optum program while residing at the Lutheran Home Memory Care Assisted Living, he or she will have access to the Optum Nurse Practitioner.
  • Memory Care Assisted Living Administrator
    • The MCAL Administrator is responsible for overall leadership and oversight of the MCAL community.
  • Care Manager
    • This individual helps coordinate move-ins and keeps families abreast of what is taking place in the community.  This will be your main contact while your loved one resides at the Lutheran Home Memory Care Assisted Living.  
  • Program Manager
    • This ambitious individual coordinates all activities and outings. There is never a dull day at the Lutheran Home’s MCAL with several activities offered each day!  Check out our latest activities (link to calendar)
  • Chaplain
    • The Lutheran Home and Harwood Place has a full time chaplain.  The Chaplain offers Sunday service, Bible Study and one-on-one visits.
  • Auxiliary Staff
    • Housekeeping – Housekeeping is included in your monthly cost.  Rooms are cleaned daily.
    • Dining Staff – Servers are present at each meal to help accommodate personal preferences.
    • Sewing Room Staff – This staff is responsible for delivering personal laundry and linens.
    • Dietary Staff – The Lutheran Home provides access to an on staff dietician.
    • Maintenance –An in-house maintenance department ensures that issues are addressed the timely.
    • Physical, Occupational & Speech Therapy – The Lutheran Home offers rehabilitation services under Medicare Parts A & B should the need arise.

Volunteers

The Lutheran Home has over 350 volunteers each year!  Our organization would not be the same without this wonderful group. The Lutheran home has a Volunteer Manager who coordinates pet therapy, volunteers to assist with activities, one-on-one visit, etc.
